Description of clinic

The GP Super Clinic is a new two storey purpose built building in West Bundaberg. There is ample car parking onsite and the location is well serviced by public transport.

Services

Multidisciplinary Care

The GP Super Clinic has GPs, nurses and allied health practitioners including diabetes education, psychology, audiometry and cardiac and pulmonary rehabilitation. Specialist practitioners associated with the Bundaberg Specialist Centre may offer sessional visitation to the Clinic subject to recruitment of additional specialist staff.

HeartSTART cardiac rehabilitation and pathology collection services are also being provided. Practice nursing and chronic disease management and rehabilitation services have commenced.

Preventative activities

Chronic disease care plans will be used at the GP Super Clinic for the early detection and management of these conditions. As part of a broad response to the underlying chronic disease load within the community, the existing HeartSTART Cardiac Rehabilitation program will be integrated into programs offered from the GP Super Clinic, and will provide a key focus on health promotion and disease prevention.

It is anticipated that as the patient base builds, additional lifestyle education activities and chronic disease self-management facilitation will be provided through the GP Super Clinic modeled upon the successful HeartSTART program. Top of page

Health needs and priorities

The local population profile reflects rates of obesity, diabetes, hypertension, and smoking that are above the prevalence level for the state of Queensland. The Clinic will have a focus on providing improved access to chronic disease management services as well as healthy lifestyle promotion and support.

Quality

The Branyan (Bundaberg) GP Super Clinic is accredited with the Royal Australian College of General Practice/ AGPAL standards.

An Executive Council will provide additional oversight of the management of the GP Super Clinic by the Medical Director, a Director of Clinical Training and the Practice Manager, and a community advisory group will be established to provide feedback on delivery of best practice standards to patients.

8am to 5pm Monday, Tuesday, Wednesday and Friday
8am to 6pm Thursday
8:30am to 12:30pm Saturday

Billing arrangements

A mixed billing approach of co-payment and bulk billing arrangements. In the November -December 2012 period, 87% of patients were bulk billed.

Bulk billing is offered to children under the age of 16, wards of the State of Queensland, Centrelink Pensioner Concession Card and Centrelink Health Care Card holders, and their dependents.

Home visits and visits to residential aged care facilities

Service provision to the aged care sector in Bundaberg will be a priority for the GP Super Clinic with the clinic working to implement scheduled hospital, nursing home and occasional home visits in the coming year.

Clinical training arrangements

GP training – Discussions are currently underway with training providers. The intention is to have GP registrars in the Clinic in 2013.

Allied Health and Nursing – Discussions are underway with several universities around placements of allied health and nursing students in the clinic. Top of page
